How to Redact Medical Records in a PDF
To redact medical records in a PDF, open a copy, mark every detail that identifies the patient, save a new file and check it by searching and copying from the saved version. In Gold PDF you press R, drag over each item and save; the marked content is removed rather than just covered.
Quick steps
- Decide who will receive the file and what they genuinely need to see.
- Make a copy and keep the original in a restricted location.
- Open the copy in Gold PDF. If it is a scan, press Ctrl+Shift+O to recognise the text first.
- Press R and drag a box over each identifier, on every page.
- Press Ctrl+S and save under a new name.
- Reopen the saved file, search for each removed value with Ctrl+F, and drag across the bars, copy and paste into Notepad.
- Check the file properties for names and clear them before sending.
What to look for in a medical record
Medical documents carry more identifying detail than most people expect, and it is scattered across many places rather than sitting in one header.
| Where | Examples |
|---|---|
| Headers and footers | Patient name, record number, page labels with initials |
| Body text | Date of birth, address, phone, next of kin, employer |
| Tables | Lab results with a patient ID, appointment lists, medication charts |
| Images | Scanned stamps, handwritten notes, ID photos, wristband labels |
| Signatures | Clinician and patient signatures and printed names |
| Hidden in the file | Author field, title, comments, original file name |
Indirect identifiers deserve attention too. A rare condition, an exact admission date and a small clinic name can together point to one person even when the name is gone. When you decide what to cover, ask what a reader could work out by combining what is left.
Why a black box does not protect a patient
A rectangle drawn on a page sits on top of the text. The text stays in the file and can be selected, copied, searched or read by assistive software, and in many editors the rectangle can be moved or deleted. For health information, that is a serious failure because the details were meant to stay private. The reasoning is explained in does drawing a black box redact a PDF.
Real redaction removes the content under the marked area when the file is saved. In Gold PDF, each page that contains a redaction is rebuilt as a high-resolution image with the bars burned in, and the remaining text is restored as an invisible layer so the rest of the page still searches and copies. The redacted values are not in that layer. See how to redact a PDF so it cannot be undone for the full picture.
Redacting scanned records
Many medical records arrive as scans or faxes, which have no text at all. You cannot search a picture, so it is easy to miss a surname in a corner or a number in a stamp. Run OCR first. Gold PDF recognises text on your own computer, with English and Hebrew working offline, and then you can search every name and number you plan to remove. Check the results against the image because handwriting and poor scans may not be read correctly. The scanned case has its own checklist in how to redact a scanned PDF document.
Deciding how much to redact
Over-redacting makes a record useless, and under-redacting exposes the patient, so settle the purpose before you start. A few common situations show how the answer changes.
- Sharing with another clinician for treatment: usually the full record is needed, so redaction may not be appropriate at all. Follow your organisation's rules.
- Sending to an insurer or employer: share only the pages and facts the request actually asks for, and remove the rest, including other people's details.
- Teaching, research or publication: remove all direct identifiers and think hard about indirect ones such as dates, locations and unusual cases.
- Responding to the patient's own request: third-party names mentioned in the record may need to be covered even though the patient is entitled to their own information.
When in doubt, remove pages entirely rather than redacting inside them. A page you do not send cannot leak, and a shorter file is quicker to check. These are general principles, not legal advice, and the right call depends on your role and local rules.
How to check the file before it leaves
Do not stop at looking at the page.
- Search for the patient's name, surname, ID and date of birth. There should be no results.
- Copy across each bar and paste into Notepad. Nothing from underneath should appear.
- Scan the whole file for repeats: headers, footers, page two of a letter, attached results.
- Check the properties for author and title. See how to remove metadata from a PDF before sharing.
- Look at the file name. "Jones-biopsy-2026.pdf" gives away what the page hides.
- Record what you shared, with whom and why, according to your organisation's process.

Sending the redacted copy
Redaction deals with what the file contains, not with how it travels. Send the tested copy only, never the working file, and use a channel your organisation approves. Where a password is appropriate, send it separately from the document. For wider data protection questions, how to redact personal information for GDPR compliance covers the surrounding duties in general terms.
A short example
A patient asks a clinic to forward a discharge summary to an insurer, with the names of two other patients in the ward list removed. The staff member opens a copy, presses R, covers both names and the matching bed numbers, and saves a new file. A search for each surname finds nothing, copying across the bars gives an empty paste, and the author field is cleared. Only then is the file attached to the message and the action noted in the record.
What Gold PDF does and does not do
Gold PDF is a free Windows PDF editor that works offline: your files are not uploaded and no account is needed. Its Redact tool (R) removes marked content on save, OCR works locally for English and Hebrew, and saving clears the file's metadata. It does not find patient data for you, so you choose every area yourself, and it cannot undo a redaction after you save, so keep a protected original. It does not make a document compliant with any law and it is not legal or medical advice. Gold PDF runs on Windows only.

Frequently asked questions
How do I redact medical records in a PDF?
Open a copy in a redaction tool, mark each identifier such as the patient's name, date of birth and record numbers, save a new file, and then search and copy from that file to confirm nothing remains underneath.
Is a black box enough to hide patient information?
No. A shape drawn over text only covers it, and the words stay in the file. Use a tool that removes the content when you save.
What details in a medical record can identify a patient?
Names, addresses, dates of birth, phone numbers, insurance and record numbers, photos, signatures, and sometimes rare diagnoses, dates of treatment or clinic names that point to one person.
Does redacting a record satisfy privacy law?
Not on its own. Rules such as HIPAA or GDPR depend on your role, country and purpose, so redaction is one technical step to use alongside your organisation's policy and advice from a qualified person.
Can I redact a scanned medical record?
Yes, but a scan is a picture. Run OCR first so you can search it for names and numbers, then redact the areas and test the saved copy.
